This is an INDIVIDUAL hospital indemnity plan paying $100, $200, or $400 per day (up to 10 days) in the hospital confinement plus an optional DENTAL, VISION and HEARING RIDER. Must be 25 years or older and each PERSON must apply separately. This is NOT a family plan. Pays up to $1000 per year after $100 deductible. You may use ANY licensed facility, no network.
